On or about April 25, 2019 two employees were working inside a 5-foot 2-1/2-inch deep unprotected trench installing sewer pipes and were not provided with a safe means of egress from the trench.

On or about April 25, 2019 two employees in an unprotected 5-foot, 2-inch deep, 3-foot 2-inch wide by 8-foot long trench were installing and sealing a new water pipe in an unprotected trench with 4 inches of ground water without dewatering equipment on site, exposing employees to trench collapse.
