Mezzanine situated adjacent to the environmental education area. The employer did not provide guardrails across a 48-foot long mezzanine, measuring 84-inches above the lower floor. Woodshop area; an energized 25-foot black heavy duty, 14/2 gauge extension cord was used a fixed wiring. Extension cord was wired directly into a four-place service box. Cord is used to power Grizzly jointer.        b. Welding area; an energized 25-foot black medium duty extension cord was wired directly to an electrical switch box.   This condition exposed employees to electrical shock and electrical burn hazards.
