29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  On or about April 29, 2019, and in times prior thereto, a fire extinguisher was not provided in the upper dog house exposing workers to a fire hazard at rig number 1.

29 CFR 1910.304(b)(3)(ii)(C)(3): Each cord set, attachment cap, plug, and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective was used before repaired;  On or abut April 29, 2019, and in times prior thereto, a connecting plug was attached to a 4 AWG cord to the top drive that had a damaged outer sheath exposing workers to an electrical hazard.
