29 CFR 1910.119(e)(1): The employer did not perform an initial process hazard analysis (hazard evaluation) on processes covered by 29 CFR 1910.119: By not conducting a PHA, the employer did not identify, evaluate and control the hazards of the process including, but not limited to: a) Pump Room; A process hazard analysis was not developed to address flow restriction or deadheading of the diaphragm pump, leading to fluid release, during transfer of normal butyl acetate and Resistovar Topcoat mixture out of the tanks, on or about, July 10, 2019. b) Pump Room; A process hazard analysis was not developed to address potential hazards of agitator misalignment leading to impact, friction, or heat from contact with tank shells or internal tank suction tubes, during periods when the agitators are operating on the tanks of normal butyl acetate and Resistovar Topcoat, on or about, July 10, 2019. c) Pump Room; A process hazard analysis was not developed to address potential hazards of seal failure on the fluid filter housings leading to uncontrolled fluid release on top of the tanks during transfer of fluids out of the normal butyl acetate and Resistovar Topcoat tanks, on or about, July 10, 2019. d) Pump Room; A process hazard analysis was not developed to address potential hazards of seal or diaphragm failure within the diaphragm pumps leading to fluid introduction inside their operating air chambers during transfer of fluids out of the normal butyl acetate and Resistovar Topcoat tanks, on or about, July 10, 2019. e) Pump Room; A process hazard analysis was not developed to address potential hazards of air pressure regulator failure leading to increased pump speed and potential fluid release during transfer of fluids out of the normal butyl acetate and Resistovar Topcoat tanks, on or about, July 10, 2019. f) Pump Room; A process hazard analysis was not developed to address potential hazards of foot actuation pedal or air solenoid failure on the remote-control circuit for the diaphragm pumps leading to loss of signal, loss of pump control, and fluid release during transfer of fluids out of the normal butyl acetate and Resistovar Topcoat tanks, on or about, July 10, 2019. g) Pump Room; A process hazard analysis was not developed to address potential hazards related to lack of readily visible instrumentation, to monitor tank agitator motor amperage and RPM, during periods when the agitators are running, leading to heat or fire at the tanks of normal butyl acetate and Resistovar Topcoat, on or about, July 10, 2019. h) Pump Room; A process hazard analysis was not developed to address potential hazards related to lack of readily visible instrumentation to monitor fluid pressures on the incoming side of the filter housings, leading to releases during fluid transfer of fluids out of the normal butyl acetate and Resistovar Topcoat tanks, on or about, July 10, 2019. i) Pump Room; A process hazard analysis was not developed to address potential hazards related to the presence of tank openings for manual gaging, leading to liquid overflow and possible vapor release of normal butyl acetate and Resistovar Topcoat, on or about, July 10, 2019. j) Pump Room; A process hazard analysis was not developed to address potential hazards related to the ability of the transfer piping and hose system to conditionally siphon fluid out of the normal butyl acetate and Resistovar Topcoat tanks, leading to fluid release, on or about, July 10, 2019. k) Pump Room; A process hazard analysis was not developed to address potential hazards related to facility siting of the normal butyl acetate and Resistovar Topcoat tanks within a recessed central floor pit that served as the drainage destination of any large fluid spill within the fire-area, on or about, July 10, 2019. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED
