An employee working inside a trench with near vertical walls was not protected from cave-in hazards, in that this employee was working in the trench installing water line connections in a trench approximately 5 feet 8 inches deep and 4 feet across while it was raining without protective systems in place. The employer did not instruct this employee in the recognition and avoidance of unsafe conditions and the regulations of trench and excavation hazards.  This condition exposed this employee to crushing, struck-by, and cave-in hazards.
