29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees.    On or about May 1, 2019, employees were exposed to cave-in hazards while performing sewer line installation on a new commercial theater located at 300 South Phelps Street, Youngstown, Ohio 44503.  The employees were working in an unprotected trench and the employer did not ensure that they had a safe means of egress within 25 feet.

On or about May 1, 2019, employees were exposed to cave-in hazards while performing sewer line installation on a new commercial theater located at 300 South Phelps Street, Youngstown, Ohio 44503.  The employees were working in an unprotected trench and the employer did not ensure that they did not undermine the pavement on both sides.
