29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  On or about May 8, 2019, and at times prior thereto, the employer failed to ensure that energy control procedures and program were developed. The employer has such equipment as, but not limited to :    1. Outside under the awning, the Cincinnati Shear, there were no energy control procedures developed. Employees were exposed to the hazard of an unexpected release of energies resulting in a caught-in hazard or an electrical shock. The Cincinnati Shear has a minimum of electrical and hydraulic and kinetic /gravitation energies.   2. Shop floor, Bradbury sheet roll forming machines, there were no energy control procedures developed. Employees were exposed to the hazard of an unexpected release of energies resulting in a  caught-in hazard or an electrical shock. The Bradbury sheet roll forming machines have a minimum of electric and pneumatic energies.   3. Shop floor, Bradbury ridge cap bending machine, there were no energy control procedures developed. Employees were exposed to the hazard of an unexpected release of energies resulting in a caught-in hazard or an electrical shock. The Bradbury ridge cap bending machines have a minimum of electric and hydraulic energies.  4. Shop floor, National shears, there were no energy control procedures developed. Employees were exposed to the hazard of an unexpected release of energies resulting in a caught-in hazard or an electrical shock. The National shears have a minimum of electric and pneumatic energies.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about May 8, 2019 and at times prior thereto, the employer failed to ensure that shop equipment being used to cut steel was guarded. Such equipment as, but not limited to:   1. Shop floor, the Bradbury roll forming machine located against the wall, the rollers were not guarded. Employees exposed to the hazard of being caught-in the rotating rollers.   2. Located outside under the awning, the Bateman Press Shear #10, was not guarded at the shear and punch locations. Employees exposed to an amputation hazard.   3. Shop floor, the National shears, the two shears were not guarded on the front or back side. Employees exposed to an amputation hazard.   4. Shop floor, the Bradbury ridge cap bending machines, the two bending machines were not guarded. Employees exposed to a caught-in hazard.   5. Shop floor, the Kalamazoo horizontal band saw, the adjustable blade guard was missing. Employees were exposed to a laceration hazard.
