29 CFR 1917.151(g)(4): Work rests on fixed grinding machines were not adjusted closely to the wheel with a maximum opening of 1/8-inch (3.18 mm) and securely clamped.    Mechanics shop adjacent to the roll-up door; on or about 06 May 2019 and times prior thereto; the work rest on a 10-inch Dayton bench grinder, model 4Z911B, serial number unknown, was not properly adjusted to a maximum of 1/8 of an inch from the abrasive wheel. The gap between the abrasive wheel and work rest measured approximately 3-inches. This condition exposed employees to a caught in hazard.

Sitka yard parking lot adjacent to mechanics garage; on or about 06 May 2019 and times prior thereto; a Manchester 420 pound propane tank used to heat the mechanics garage, was not protected from vehicular traffic. This exposed employees to contact with flammable and combustible liquids and other fire hazards.

29 CFR 1917.112(e)(1): Stairways with four or more risers and less than 44 inches wide were not equipped with at least one railing.  Warehouse dock; on or about 06 May 2019 and time prior thereto the employer did not ensure that stairways with four or more risers' and less than 44 inches was equipped with a handrail. Two instances were noted:       a. Warehouse dock loading ramp right hand side access; a set of stairs approximately 28 inches wide with seven risers' did not have a hand rail installed.       b. Warehouse dock loading ramp center access; a set of stairs approximately 30 inches wide with seven risers' did not have a hand rail installed.  This condition exposed employees to a fall hazard.
