29 CFR 1926.452(w)(6)(i): An employee was allowed to ride on scaffolds when the surface on which the scaffold was moved was not within 3 degrees of level, and/or not free of pits, holes, and obstructions:    12000 W 52nd St, Wheat Ridge: On or about April 16, 2019 a worker was assigned to operate a scissor lift at the exterior of a building with uneven, sloped pavement, an abrupt edge drop, and a vehicle parked in close proximity to the area of scissor lift operations.    Abatement Note: Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet).

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  a) 12000 W 52nd St, Wheat Ridge: On or about April 16, 2019, and at times prior, two or more workers were exposed to hazards while operating a JLG scissor lift prior to being trained to correct or avoid hazards during operations. 29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye:    a) On or about April 16, 2019, a worker sustained an on-the-job injury resulting in hospitalization which was not reported to OSHA by any means within twenty-four hours.    Abatement Note: Abatement certification is not required.  The incident was reported by a third party on May 6, 2019.
