29 CFR 1910.28(b)(3)(iv): The employer did not ensure each employee was protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toe boards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used.  a) On or about 06/07/19 at pug mill #5; employees were exposed to an unprotected fall of 60 inches when the ladderway opening at the top of the work platform was open and on either side of the ladderway was open.       b) On or about 06/07/19 at Handle wad mill #8 ; employees were exposed to an unprotected fall of 50 inches when the ladderway opening at the top of the work platform was open and on either side of the ladderway was open.      c) On or about 08/08/19 in the Cementing area; employees were exposed to an unprotected fall of eight feet from the mezzanine, when the work platform opening to the access ladder was open.       d) On or about 08/08/19 in the Cementing area; employees were exposed to an unprotected fall of seven feet when the opening on the upper mezzanine was protected by two chains attached on either side.     NO ABATEMENT CERTIFICATION REQUIRED
