29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   (a) Location - 18 Brooklawn Terrace, Lynn, MA:  On or about May 8, 2019, employees were exposed to fall hazards where a portable ladder did not extend at least 3 feet above the upper landing.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) Location - 18 Brooklawn Terrace, Lynn, MA:  On or about May 8, 2019, employees were exposed to 22-foot fall hazards while installing shingles to a new roof without fall protection.  The Lozzi-Merrill Framing Company was previously cited for a violation of this occupational safety and health standard 1926.501(b)(1), which was contained in OSHA inspection number 1310451, citation number 1, item number 2 and was affirmed as a final order on August 7, 2019, with respect to a workplace located at 1064 N. Shore Road, Revere, MA.  The Lozzi-Merrill Framing Company was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1150136, citation number 1, item number 2 and was affirmed as a final order on December 12, 2016, with respect to a workplace located at 485 Beach Street, Revere, MA.
