29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems: On or about May 20, 2019, employees were exposed to hazards associated with cave-ins while working in and around a trench that had not been inspected by a competent person. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c)

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about May 20, 2019, employees installing a water main were not protected from a cave-in by an adequate protective system, exposing the employees to the hazards associated with a cave-in. The trench the employees were working in was approximately 6 feet deep. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d)
