29 CFR 1910.110(f)(2)(i): Containers in storage were not located so as to minimize exposure to excessive temperature rise, physical damage, or tampering by unauthorized persons.  Storage area behind wood shop; on or about 22 May 2019 and times prior thereto; the employer did not ensure that propane fuel cylinders were stored in a manner to prevent damage. Sixteen propane cylinders of various quantities, were stored free standing. This condition exposed employees to contact with flammable and combustible liquids and other fire hazards.

Wood shop; on or about 22 May 2019 and times prior thereto; a Milwaukee seven-inch bench mounted grinder, model 4991, serial number W4500; did not have the work rest adjusted to less than one-eighth of an inch from the abrasive wheel. The distance from the work rest to the abrasive wheel was approximately one-quarter of an inch. This condition exposed employees to a caught in hazard.

Wood shop; on or about 22 May 2019 and times prior thereto; a Milwaukee seven-inch bench mounted grinder, model 4991, serial number W4500; did not have the tongue guard adjusted to less than one-quarter of an inch from the abrasive wheel. 29 CFR 1910.253(b)(2)(iv): Valve protection caps, where cylinders were designed to accept caps, were not always in place, hand-tight, except when cylinders were in use or connected for use.  Garage bay, on or about 22 May 2019 and times prior thereto; the employer did not ensure that protective valve caps were installed on cylinders when not in use. An oxygen acetylene welding cart had the regulator removed. The tanks did not have protective valve caps installed. This condition exposed employees to fire and explosion hazards.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket).  Garage and wood shop; on or about 22 May 2019 and times prior thereto; the employer did not ensure that portable cord and plug equipment was inspected prior to use. Three instances were noted:       a. Garage bay; a 50-foot heavy duty blue extension cord was missing the ground pin.       b. Wood shop; the power cord to the Jet Tilting Arbor Saw, model JTAS-10-1, serial number 6083284 had damage to the outer protective jacket next to the cord plug.       c. Wood shop; a black 25-foot heavy duty extension cord used to supply power to the arbor saw had damage to the outer protective jacket next to the cord plug.   This condition exposed employees to electrical shock and electrical burn hazards.

Garage bay; on or about 22 May 2019 and times prior; the employer did not ensure that safety data sheets for each hazardous chemical located in the workplace was readily accessible. Random samples of chemicals were selected. Three instances were noted:       a. CRC Clean-R-Carb, Carburetor Cleaner, 16 ounce aerosol can.            b. Super Clean, Cleaner-Degreaser 32 ounce spray bottle.        c. PB Blaster Penetrating Oil, 11 ounce aerosol can.  This condition exposed employees to chemical burns, irritation of the eyes and upper respiratory tract, and skin dermatitis hazards.

Administrative Building; on or about 22 May 2019 and times prior thereto; employees were not trained on the new Hazard Communications Standard (HCS 2012) provisions implemented on 1 December 2013. This training includes revised labeling elements and the new format for Safety Data Sheets. This condition exposed employees to inadequate training and chemical hazards.
