29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a.    Roof top, 2175 Marlton Pike Rd West, Cherry Hill:  Employees not using fall protection, and exposed to an approximate 18 foot fall, were installing new 5G cell phone antennas and hooking up system while on the top of a walled in mechanical area, on top of the Holiday Inn roof, on or about 06/03/2019.  NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:(a) (LOCATION) (IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS)   a)  Roof top mechanical area, 2175 Marlton Pike Rd West, Cherry Hill: Employee used a Werner Type 1A Multi-Position ladder to get down from the approximate 18 foot high wall; ladder was only extended approximately 11-12 feet; employee climbed down through the various sections of conduit while holding onto the support posts for the antennas mounted on the upper part of the wall and the horizontal conduits when needed to access the ladder, on or about 06/03/2019.    NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
