29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) Douglas Barrrels, Drill Room - the rotating chuck on the Jet lathe, Model Number 37-1500, Serial Number 1060, was not equipped with a guard for the rotating chuck and to contain flying chips.                 (b) Douglas Barrrels, Barrel Fitting Room - the rotating chuck on the Turnmaster 13x40 Lathe, Model Number TRL1840 Serial Number 13490021458, was not equipped with a guard for the rotating chuck and to contain flying chips.         (c) Douglas Barrrels, Barrel Fitting Room - the Clausing Drill Press Model Number Series 16VC-1, Serial Number 511417, was not provided with a guard to prevent employee exposure to flying chips and point of operation hazard.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  (a) Douglass Barrels, Drill Room - the work rest installed on the left side of the Dayton pedestal abrasive wheel combination grinder/horizontal belt sander/disc sander Serial Number M40000-B, Model Number I7706-B, was approximately 1/4 inches from the abrasive wheel.    (b) Douglass Barrels, Barrel Fitting Room - the work rest installed on the left side of the Dayton pedestal abrasive wheel combination grinder/horizontal belt sander/disc sander Serial Number 63923, Model Number 1Z853, was approximately 1/4 inches from the abrasive wheel.    (c) Douglass Barrels, CNC Room - the work rest installed on the left side of the Dayton pedestal abrasive wheel combination grinder/horizontal belt sander/disc sander Serial Number MMT40000AKV, Model Number 1Z706, was approximately 1/4 inches from the abrasive wheel.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  (a) Douglas Barrrels, Drill Room - the Dayton abrasive wheel pedestal grinder, Serial Number M40000-B, Model Number I7706-B.  The right side of the grinder did not have the tongue guard adjusted with 1/4 inches from the abrasive wheel.  The opening was approximately 1 inch.  (b) Douglas Barrrels, Barrel Fitting Room - the Dayton abrasive wheel pedestal grinder, Serial Number 63923, Model Number 1Z853.  The right side of the grinder did not have the tongue guard adjusted with 1/4 inches from the abrasive wheel.  The opening was approximately 1/2 inch.  (c) Douglas Barrrels, CNC Room - the Dayton abrasive wheel pedestal grinder, Serial Number  MMT40000AKV, Model Number 1Z706.  The right side of the grinder did not have the tongue guard adjusted with 1/4 inches from the abrasive wheel.  The opening was approximately 1/2 inch.

29 CFR 1910.253(b)(4)(iii):  Oxygen cylinders in storage shall be separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour.  (a) Douglass Barrels, Barrel Fitting Room - 2 oxygen cylinders and 2 acetylene cylinders were not separated by a minimum distance of 20 feet or by a noncombustible barrier at least 5 feet high having a fire-resistance rating of at least one-half hour.
