29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:  a) Diversified Masonry Contractors, LLC at 355 Eldorado Blvd in Broomfield, CO 80021: On or before June 3, 2019, the employer did not ensure that employees were able to access the 5th floor of scaffolding through the use of ladders, hook on ladders, or any other compliant means of access. An employee was seen climbing the cross bracing of the scaffolding greater than 40 feet off of the ground to gain access to the 5th floor of scaffolding . This condition exposed the employee to fall from height hazards.
