The information in Column F was not recorded in the detail required as it did not describe the object that directly injured the employee.     b) On or about May 31, 2019, a recordable injury occurred when a spot welder pressed the foot pedal of an unguarded spot weld machine while his hand was in the point of operation, resulting in the amputation of the employee's right index finger.  The information in Column F was not recorded in the detail required; the object that directly injured the employee was not described nor was the injury accurately described.
