29 CFR 1910.255(b)(4):  All press welding machine operations, where there was a possibility of the operator's fingers being under the point of operation, were not effectively guarded by use of a device such as an electronic eye safety circuit, two hand controls or protection similar to that prescribed for punch press operation in 29 CFR 1910.217:      a.)  Production Floor - On or about June 5, 2019, the employer failed to effectively guard a foot-switch operated spot welding machine.  Spot welding machine #2 had an unguarded space of more than one (1) inch between the bottom and top electrodes, thereby exposing employees to crushed-by and/or amputation hazards.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.   a) On or about May 31, 2019, the employer did not contact OSHA within 24 hours when an employee sustained a work-related injury that resulted in an amputation and in-patient hospitalization.

29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in detail as required by the regulation:    Hohmann & Barnard, Inc. - Chicago Ridge, IL - On or about June 5, 2019, the employer failed to record the following workplace injuries in the detail required by the regulation on the OSHA 300 Log for calendar year 2019.    a) On or about March 13, 2019, a recordable injury occurred when a spot welder pressed the foot pedal of an unguarded spot weld machine while his hand was in the point of operation, resulting in a cut to the employee's left thumb which required stitches.  The information in Column F was not recorded in the detail required as it did not describe the object that directly injured the employee.     b) On or about May 31, 2019, a recordable injury occurred when a spot welder pressed the foot pedal of an unguarded spot weld machine while his hand was in the point of operation, resulting in the amputation of the employee's right index finger.  The information in Column F was not recorded in the detail required; the object that directly injured the employee was not described nor was the injury accurately described.
