29 CFR  1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  a.) Production Floor - On or about December 7, 2020, an authorized employee performed a service and/or maintenance task on the MTech Ladder Wire Machine, Model 00, Serial No. 8.  The employer did not ensure a periodic inspection of the energy control procedure was performed at least annually as required by the standard.

1910.147(f)(1)(ii):  Remove employees from the machine or equipment area in accordance with paragraph (e)(2) of this section;  a. On or about December 7, 2020, an employee was positioning the MTech Ladder Wire Machine, Model 00, Serial No. 8 to load wire. The employer did not ensure that the employee was clear of the machine by using an extension tool to load the wire while the machine was energized.
