29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying existing hazards.)     a) Jobsite:     The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person. Employees were exposed to hazards, including but not limited to: fall from working at an unprotected edge 7 feet above the lower level without fall protection, struck by cinder blocks sliding down on an extension ladder from a higher level, and fall from sliding down on an extension ladder from a higher level. On or about 06/11/19.

29 CFR 1926.501(b)(1):  Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.     a) Ground floor, south side:     Employees were transporting cinder blocks from the ground floor to the basement level by sliding them down wood planks placed on an extension ladder. Employees were working at the edge of a ground floor opening without fall protection and were exposed to falling approximately 7 feet to the basement level. On or about 06/11/19.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed.  a) Ground floor, south side:  Employees placed wood planks on an extension ladder that accessed the basement level and slid cinder blocks down the planks to transport them. An employee also slid himself down the planks to access the basement level. On or about 06/11/19.
