29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.   A) Exterior on the ground: Employees were exposed to head injuries while working below an employee above in a lift. The employees on the ground were not protected by hard hats or protective helmets, on or about November 21st, 2019.

29 CFR 1926.453(b)(2)(v):   A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift.   A) Exterior, in lift:  An employee was working in a Skyjack lift and was not tied off.  The employee was exposed to fall/catapult hazards of approximately 8 feet, on or about November 21st, 2019.
