29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift.  On or about 20 June 2019, and at times prior thereto, three roofers used an aerial lift to access a roof without being tied off within the basket, exposing the workers to a fall hazard of approximately 24 feet 6 inches to a concrete apron below.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.  On or about 20 June 2019, and at times prior thereto, three roofers were installing insulation and standing seam metal roofing without fall protection, exposing the workers to a fall hazard from approximately 24 feet 6 inches up to 29 feet 10 inches to a concrete apron below.
