(a) On or about June 28, 2019,  at the road construction site located on (State Hwy 11) Durand Ave. near the Ohio St. intersection, Racine, WI, employees were exposed to cave-in hazards while conducting pipe repair work in an unprotected trench. The competent person determined that the trench required cave-in protection, but failed to have the trench protection installed where employees were exposed to cave-in hazards nor had a sufficient number of  egress points installed in the trench for employees to exit the trench safely.
