Employees were not provided a safe means of egress, a ladder, located on the east side inside the trench. Employees were climbing in and out of the trench.  The Holtger Bros., Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.651(c)(2), which was contained in OSHA inspection number 1411959, citation number 1, item number 1a and was affirmed as a final order on 01/15/2020, with respect to a workplace located at Durand Ave. and Ohio St. Racine, WI 53403.
