On or about June 27, 2019, at the above addressed jobsite, an employee using a leaf blower did not wear safety glasses, thereby exposing the employee and other roofers working in close proximity to the hazards associated with eye injuries.

On or about June 27, 2019, at the above addressed jobsite, employees performing residential roofing operations on a low sloped roof (4:12 or less), nine (9) feet seven (7) inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.
