29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:    (a)L L & Sons Excavation, Inc. near the intersection of 9th Street Southeast and South Washington Avenue in Madison, SD 57242: On or about and at times prior to July 1, 2019, the exposing employer did not ensure employees were protected from cave-in hazards in that the a appropriate means of safe egress was not located in the trench excavation greater than 4 feet in depth. This condition required employees to use the trench walls to access and egress from the excavation. This condition exposed employees to cave-in hazards.

(a)L L & Sons Excavation, Inc. near the intersection of 9th Street Southeast and South Washington Avenue in Madison, SD 57242: On or about and at times prior to July 1, 2019, the exposing employer did not ensure employees were protected from cave-in hazards in that employees performing work in multiple areas of the excavation did not have adequate protective systems designed in accordance with 29 CFR 1926.652 (b). This condition exposed employees to cave-in hazards.

This condition exposed employees to cave-in hazards.
