29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  On or about July 12, 2019, and at times prior thereto, the employer did not ensure employees used fall protection attached to the basket anchor point when operating and elevated in a Genie 4x4 aerial lift. The employees were exposed to ejection and fall hazards up to 40 feet.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  On or about July 12, 2019, and at times prior thereto, the employer did not ensure employees performing roofing activities used appropriate fall protection including lanyards tied off to anchor points, exposing the employees to an unprotected fall hazard of approximately 40 feet to the ground below.
