29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b):  West side of the residential construction site, building 7: On or about July 15, 2019, and at times prior thereto, employees without fall protection were exposed to a fall hazard greater than 6 feet to the unpaved ground below while installing the joists on the roof.

29 CFR 1926.502(k)(1): The fall protection plan did not conform to the provisions specified in 1926.502(k)(1) through (k)(10):   Instances: (a) failure to identify each location where conventional fall protection methods cannot be used, and classifying these locations as controlled access zones; (b) failure to implement a safety monitoring system when no other alternative measure had been implemented; and (c) failure to include a statement that provides the name or other method of identification for each employee who is designated to work in controlled access zones. West side of the residential construction site, building 7: On or about July 15, 2019, and at times prior thereto, employees were exposed to a fall hazard, without a written fall protection plan that conformed to the regulatory requirements.
