29 CFR 1910.28(b)(3)(iv): Each employee was not protected from falling into the ladderway platform hole at the entrance to the hole by a self-closing gate or offset.    On or about July 9, 2019, at Pratt Energy, LLC., near the platform located inside Scoular Grain's receiving building, in Pratt Kansas, employees tracing security camera conduit lines were exposed to fall hazards of approximately twenty-eight (28) feet to the platform below in that the entrance to the ladderway was not protected by a functional self-closing gate.      29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is complete.
