29 CFR 1910.147(c)(5)(ii)(D): Lockout devices and tagout devices did not indicate the identity of the employee applying the device(s):    Employees engaged in servicing and maintenance activities throughout the entire plant were exposed to electrical, struck-by and caught-between hazards in that lockout devices and tagout devices did not indicate the identity of the employee applying the device, on the following equipment:    a) EP-2302 Mash Cooler (A) Air by the Sump area of the Cook Building,  b) Condensate Flush Line, and   c) PM-8510 A box by the acid pump.

29 CFR 1910.147(c)(6)(ii): The employer did not certify that the periodic inspections have been performed:    Employees engaged in servicing, and maintenance activities throughout the entire plant at the following equipment but not limited to the Beer Mash Exchanger A Box #2, the PC-4113 Pre-Heater Plush Pump Box #12, the Paddle Pre-Mixer BP-5401, the mixer, the Acid pump, the blend enzyme tank pump, the Cook Tube Seal Box #14, and the Hammermill Box #22, which had multiple energy sources including pneumatic, electric, hydraulic, and gravity were exposed to electrical, struck-by and caught-between hazards in that periodic inspections of the energy control procedures for equipment were not performed.
