29 CFR 1904.29(b)(3): The employer did not enter each recordable injury or illness on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness had occurred:  YMCA - The employer did not record an injury or illness on the OSHA log that occurred on July 18, 2019 that resulted in an employee being transported to the hospital by an ambulance, on or about July 22, 2019.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1904.40(a): The employer did not provide an authorized government representative the records within the four business hours.  YMCA - The employer failed to provide copies of the injury and illness report (OSHA 300 logs) to OSHA within four business hours of the request, on or about September 5, 2019.   Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
