A.)  Preferred Roofing Services, LLC, 2310 Pittsburgh Avenue, Erie, PA  16502, on or about July 24, 2019 and times prior thereto:  Employees were utilizing a warning line system on the upper roof of the structure when performing roofing activities.  The warning line was sagging down and was within 12-inches of the roof surface.

Preferred Roofing Services, LLC, 2310 Pittsburgh Avenue, Erie, PA  16502, on or about July 24, 2019 and times prior thereto:  Employees were utilizing a warning line system on the upper roof of the structure when performing roofing activities.  A rope, wire, chain, or other barricade was not placed across the pathway to/from the fixed ladder.  In addition, the path was not offset such that a person cannot walk directly into the work area.

Preferred Roofing Services, LLC, 2310 Pittsburgh Avenue, Erie, PA  16502, on or about July 24, 2019 and times prior thereto:  Employees were gaining access to / egressing from a middle roof via a set of fixed stairs and a parapet wall.  The break in elevation for the middle roof to the top of the parapet wall was 21-1/2-inches.
