29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: The employer is failing to protect employees from fall hazards when conducting roofing operations on a residential structure. This was most recently documented on or about July 25, 2019 at 9931 Pasadena Avenue, Omaha, NE 68124 when employees were observed installing shingles on a residential structure while working at greater than 6 feet above the ground without fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: The employer is failing to train all employees that were potentially exposed to fall hazards to enable each employee to recognize the hazards associated with the type of work. This was documented on or about July 25, 2019 at 9931 Pasadena Avenue, Omaha, NE 68124. This should include training on mitigating those hazards through an acceptable method of fall protection. Training should also include the requirements for fall protection systems and when it is appropriate to utilize those systems.
