29 CFR 1918.64(b): All conveyor and trimmer drives that create a hazard were not adequately guarded.    F/T Northern Jaeger, Mid-ship port; on or about 29 July 2019 and times prior thereto; the employer did not ensure that a rotating shaft was effectively guarded. The fish meal conveyor, make and model unknown, did not have the protective guard installed. This condition exposed employees to a caught in hazard.

29 CFR 1904.32(a)(1): The employer did not review the OSHA 300 log to verify that entries were complete and accurate, and correct any deficiencies identified.    F/T Northern Jaeger - Administrative office; on or about 29 July 2019 and times prior thereto; the company executive did not ensure that the OSHA 300A Summary of Work-related Injuries and Illness Log was accurate and complete, and/or deficiencies were identified and corrected at the end of the calendar year. Two instances were noted:         a. 2018 OSHA Form 300A - Total Recordable Cases (J) is different from Total of Injuries and Illnesses (M).     This condition leads to inaccurate reporting of injuries and illnesses.
