A full-face respirator was observed in use and stored out in the open within an area with a consistent concentration of airborne formaldehyde. A half face respirator was observed hanging from an employee's neck with visual contamination on the interior.

Access to electrical panels labeled "#2 Parajust", "Glycol Cooling", "Press" and "Main Power Panel 1" were blocked by multiple large, immovable, equipment. The blocking equipment includes but it is not limited to: large storage bags, drums filled with dust and other items, a permanently installed guard rail, and a hand truck.
