29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals:    Throughout the facility at 238 Nonotuck Street in Florence, MA, on and before August 2, 2019, employees required to wear tight-fitting respirators did not store them in a manner that would protect from contamination, dust, damaging chemicals and other hazards.  A full-face respirator was observed in use and stored out in the open within an area with a consistent concentration of airborne formaldehyde.  A half face respirator was observed hanging from an employee's neck with visual contamination on the interior.

Access to electrical panels labeled "#2 Parajust", "Glycol Cooling", "Press" and "Main Power Panel 1" were blocked by multiple large, immovable, equipment.  The blocking equipment includes but it is not limited to:  large storage bags, drums filled with dust and other items, a permanently installed guard rail, and a hand truck.
