29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:    a) Opposite 156 Northampton St., Southampton, MA: On or about August 6, 2019, an employee was observed using an aluminum extension ladder that did not extend 3 feet past the upper landing surface ( top sill of wall being framed) he was accessing to perform his work.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:    a) Opposite 156 Northampton St., Southampton, MA: On or about August 6, 2019, an employee was observed installing insulated roof panels (to create the roof of this structure) without any fall protection more than 12 feet above the ground below.     Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19(d)(1).
