The employee was exposed to a fall distance of 12 feet to the ground level and eye injuries due to the lack of using eye protection.    Chris Gaskill was previously cited for a violation of this Occupational Safety and Health Standard, which was contained in OSHA inspection number 1330903, citation number 1, item number 1a and was affirmed as a final order on September 20, 2018, with respect to a workplace located at 7231 Monarch Street, Canal Winchester, Ohio.
