Roofers were removing/installing shingles on a roof that varied from 10-21 feet with the employees working at different levels. The employees were not utilizing proper fall protection which exposed them to falls up to 21 feet.

The employer did not initiate and maintain safety and health programs related to the roofing work being conducted by employees.

Roofers were removing/installing shingles on a roof that varied from 10-21 feet with the employees working at different levels. The employees were not utilizing proper eye, head or fall protection which exposed them to falls up to 21 feet and struck by hazards to the head and eyes.

Roofers were installing roofing shingles utilizing a pneumatic nail gun set at 120 psi and were not utilizing eye protection which exposed the them to struck by hazards to the eyes.
