Located at 78 N Hempstead Rd, Westerville, OH 43081: On or about November 21, 2025, the employer failed to ensure employees were protected against fall hazards of approximately 9 feet while performing re-roofing on a residential home.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  a. Located at 78 N Hempstead Rd, Westerville, OH 43081: On or about November 21, 2025, the employer did not provide a fall protection training program to ensure that employees engaged in roofing activities on a residential home, were able to recognize the hazards of working at heights of approximately 9 feet.

29 CFR  1926.1053(b)(6):Ladders shall be used only on stable and level surfaces unless secured to prevent accidental displacement.  a. Located at 78 N Hempstead Rd, Westerville, OH 43081: On or about November 21, 2025, workers performing re-roofing activities were using an extension ladder on an unstable surface and unsecured against displacement. The ladder was setup on the ridge of a sloped roof.
