29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent and continuous:       (a)   Youree Family LLLP dba Riverview Park and Campgrounds at 2444 River Rim Road, Loveland, CO 80537:  On and before August 20, 2019, the employer did not ensure that the path to ground for the electric submersible water pump was continuous.  The extension cord used to connect the pump to a portable generator had a missing ground pin. The extension cord was also not plugged into a GFCI-protected outlet. This condition exposed employees to electric shock hazard.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected electric equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):    (a)   Youree Family LLLP dba Riverview Park and Campgrounds at 2444 River Rim Road, Loveland, CO 80537:  On and before August 20, 2019, the employer did not ensure that electrical equipment was visually inspected before each use in that employees were connecting an extension cord plug end that had a missing ground pin to a portable generator each time they needed to use the submerged water pump to fill the tank on the water truck. This condition exposed employees to electric shock hazard.
