29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s):    On or about September 11, 2019, in the fabrication area, on the west side of the shop, where an employee was exposed to a struck-by hazard when operating a Cincinnati (brand, no serial number) 20-10 press break without point of operation guard.    Houston Fab & Truck Rigging, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1014034, citation number 1 item number 3c and was affirmed as a final order on January 22, 2015, with respect to a workplace located at 1124 Keyport Ln, Houston, TX 77015.
