29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. On or about April 02, 2019 and at times prior thereto, the employer did not ensure that each employee working from a roof with unguarded skylights were protected from falling. This exposed the employees to a fall hazard greater than six(6) feet to the concrete ground below.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system On or about April 02, 2019 and at times prior thereto, the employer did not ensure that the employees working from a low slope roof with unprotected sides and edges were protected from falling by using a fall protection system. This exposed the employees to a fall greater than six(6) feet to the concrete ground below.
