On 9/10/19, in the warehouse area where employees refill forklift batteries with water, an eye wash was not available for immediate emergency use and instead was located in a different area of the warehouse.

On 9/10/19, in the warehouse area, temporary electrical extension cords were used in lieu of permanent wiring.
