29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage shall be separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour.      On or about September 5, 2019, an Oxygen cylinder was stored within 20 feet of a fuel-gas Acetylene cylinder in the area under the sawmill.

29 CFR 1910.1200(f)(6)(ii): Workplace labeling.  Except as provided in paragraphs (f)(7) and (f)(8) of this section, the employer shall ensure that each container of hazardous chemicals in the workplace is labeled, tagged or marked with product identifier and words, pictures, symbols, or combination thereof which provide at least general information regarding the hazards of the chemicals.    a.  On or about September 5, 2019, the employer did not ensure that a container of chemicals used to treat the boiler was labeled, tagged or marked with product identifier and words, pictures, symbols, or combination thereof.  The plastic container had a mixture of chemicals such as Sodium Hydroxide and Diethylaminoethanol, which the employees would use to prevent scale buildup in the boiler system.    b.  On or about September 5, 2019, the employer did not ensure that a plastic container of diesel fuel used to lubricate the Swecan headsaw was labeled, tagged or marked with product identifier and words, pictures, symbols, or combination thereof.

29 CFR 1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.      On or about September 5, 2019, the employer was unable to verify that a workplace hazard assessment had been performed for employees engaged in job tasks such as operating the Debarker, Headsaw, Edger, and Resaw.
