29 CFR 1910.28(b)(1)(i): (1) Unprotected sides and edges.  (i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following:    On or about 09/24/2019, at the above addressed jobsite located at 6714 Forest Hill Blvd, Greenacres FL 33413, an employee was not protected from falling when working in the near designated area at approximately 20 feet above ground level without fall protection, exposing him to a fall hazard.

29 CFR 1910.67(c)(2)(v):  A body belt was not worn and lanyard was not attached to the boom or basket when working from an aerial lift.  On or about 09/24/2019 at 6714 Forest Hill Blvd, Greenacres FL 33073, employees were not protected from falling while working in an aerial lift at a height of 20 feet from ground level.
