29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.    a) On or about 10/07/19, on a roofing job site, at a house at 505 Norwood Ave., Buffalo, NY: One employee was doing shingling & flashing work near the chimney on the low slope roof of the dormer on the north side of the house. The employee was exposed to a fall of approx. 30 ft. because no fall protection system was being utilized. The pitch of the roof in that area was approx. 4 in 12.  b) On or about 10/07/19, on a roofing job site, at a house at 505 Norwood Ave., Buffalo, NY: One employee was engaged in roofing work on the low slope roof of the dormer on the south side of the house. The employee was exposed to a fall of approx. 30 ft. because no fall protection system was being utilized. The pitch of the roof in that area was approx. 4 in 12.  NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On or about 10/07/19, on the roofing jobsite at a house at 505 Norwood Avenue, Buffalo, NY: An extension ladder was set up on the driveway on the north side of the house and used by an employee to access the pik on a ladder jack scaffold. The siderails of that extension ladder were only approx. 15 inches above the aluminum pik.  b) On or about 10/07/19, on the roofing jobsite at a house at 505 Norwood Avenue, Buffalo, NY: An extension ladder was set up on the ground on the south side of the house and used by an employee to access the balcony. The siderails of that extension ladder were only approx. 13 inches above the balcony landing.  c) On or about 10/07/19, on the roofing jobsite at a house at 505 Norwood Avenue, Buffalo, NY: An extension ladder was set up on the balcony on the south side of the house and used by an employee to access the low slope dormer above it. The siderails of that extension ladder were only approx. 12 inches above the roof.  NO ABATEMENT CERTIFICATION REQUIRED
