29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Wrought Design, LLC located at 105 E. Chestnut Street, Mount Vernon, OH 43050:  On and preceding October 15, 2019, the employer did not ensure that ladders used for access to an upper landing surface extended at least three feet above the upper landing being accessed.  Roofer were using an extension ladder to access to top of an awning which extended approximately 6 inches beyond the edge of the awning they were accessing which was 13 feet high with nothing securing it to the awning.  The following condition exposed employees to falls up to 13 feet.
