Location:  Building 7, 225 Locust St., Woburn, MA On or about October 16, 2019, the employer's competent person did not conduct adequate frequent and regular inspections of the jobsite to identify the lack of: fire extinguishing equipment; energized electrical cord defects; lack of stair rails/handrails on stairways; non-uniformed stair riser heights; untrained and non-evaluated powered industrial truck operators; inadequate use of fall protective equipment; and the use of automotive tow straps as rigging slings.
