29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s).      a) On or about October 17, 2019, in the Maya Structure-I at Shell Deer Park Refinery, the employer failed to ensure all isolating devices needed to isolate the 200# steam system were operated in a manner that isolated the equipment from all energy sources.

29 CFR 1910.147(d)(5)(ii) : When there was a possibility of reaccumulation of stored energy to a hazardous level, verification of isolation was not continued until the servicing or maintenance is completed, or until the possibility of such accumulation no longer exists.    a)  On or about October 17th 2019, in the Maya Structure-I at Shell Deer Park Refinery, the employer did not recognize the possibility of re-accumulation of energy and continue verification of isolation.
