29 CFR 1926.251(b)(1): Welded alloy steel chain sling(s) did not have permanently affixed durable identification stating size, grade, rated capacity, and sling manufacturer:   On or about October 18, 2019, at the construction site of a new Flix Brewhouse, employees used a spreader bar with unmarked steel alloy chains. Employees were exposed to crush-by and/or struck-by hazards.

29 CFR 1926.1417(e)(1): The crane operator left the controls while the load was suspended and did not meet all of the requirements of (i) through (iv) of this standard.  On or about October 18, 2019, at the construction site of a new Flix Brewhouse, employees left a Link-Belt HC-268 250 Ton Crane load unattended. The employer did not ensure the load was secured and barricaded. Employees were exposed to crush-by and/or struck-by hazards.

29 CFR 1926.1424(a)(2)(ii): To prevent employees from entering hazard areas, the employer did not erect and maintain control lines, warning lines, railings or similar barriers to mark the boundaries of the hazard areas and/or did not train each employee to understand what these markings signify.  On or about October 18, 2019, at the construction site of a new Flix Brewhouse, employees used a Link-Belt HC-268 250 Ton Crane that was not equipped with a swing radius barrier. Employees were exposed to crush-by and/or struck-by hazards.
