A) ENTERPRISE ROOFING & REMODELING SERVICES, INC. on or about October 29, 2019 at the address of 5860 Cresthaven, Toledo, OH: The employer did not ensure employees were protected from falls while installing new ice guard and tar paper to the residential structure. The employees were exposed to a potential falls of 40 - 50 feet to the ground below.
